First Visit

Our goal for this visit first and foremost is to have fun as we get to know one another!  Here's the perfect opportunity for you to share all of  your concerns, anxieties, ideas, excitement and goals for your dental health.  A complete and thorough examination will be performed by Dr. Gianino. Other information gathered will include digital x-rays, digital photos, and if necessary study casts of your mouth. The information you shared along with all of the clinical findings will provide the best opportunity for a customized dental care program to be developed.

Please assist us by providing the following information at the time of your first visit:

  • A complete list of medications that you are presently taking.
  • A list of dental concerns and/or cosmetic desires.
  • If time allows, a completed health history/registration form ( this can be downloaded from our contact us tool bar). This form can also be filled out at your first visit, which ever works best for you! 

IMPORTANT: A parent or guardian must accompany all patients under 18 at the first visit.

Please alert the team if you have a medical condition that may be of concern  (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings, periodontal exams or dental surgery (i.e. antibiotics, for pre-med).

X-Rays

If your previous dentist has taken quality x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they will be taken at our facility. If digital x-rays were taken, request they place the images on a CD.
